Description:
Bridgeview Center located in Ormond Beach, Florida is seeking an energetic individual for our Care Plan Coordinator. He/she reports to the Clinical Reimbursement Coordinator Responsible to determine the customer’s acuity level and document in the customer’s Medical Record. Assessment of all new admissions as well as assume responsibility for all levels of care changes. Participate in communicating changes of acuity levels of customers to the Medical Director, Administrator, Admissions Director and DON. Coordinate participation in completing the MDS within 14 days for new admissions as well as completing the RAP form within guidelines. Maintain accurate records of customer assessments and coordinate the completion of Plan of Care. Must have current State RN license, comprehensive knowledge of Medicare reimbursement, Medicaid and third party payor regulations.. Minimum of 3 years Clinical experience and 2 year long term care clinical nursing experience is required.
