Description:
Concordia Care, a Program of All-inclusive Care for the Elderly (member National PACE Association), whose mission is to provide an alternative to nursing home placement by enabling seniors to remain in the community. CHIEF OPERATING OFFICERIncumbent will be responsible for the development, planning, implementation and evaluation of the Concordia Care PACE program and multiple community contracts to cover all aspects of care for frail elderly participants on a 24 hour basis. Responsible for the administration and oversight of all clinical service operations, including the primary care clinic and home care. Directs the Infection Control program. Responsibilities: * Ensures departmental compliance with PACE regulations, industry standards and Concordia Care policies * Manages department human resources by: determining qualifications, competence and performance expectations; planning, implementing and monitoring staffing plans; providing orientation, in-service training and continuing education of all persons in the department and applying policies/procedures * Oversees Transportation resources to support participant service delivery * Oversees and manages the medical records function for the agency, including: maintenance, storage and retrieval of all participant information * Provides effective leadership by coordinating and integrating services within the Clinic and Adult Day Health Center including home health care, hospital and community contracts * Manage the environment (of care) by developing, implementing and monitoring processes to reduce and control environmental hazards and risks; preventing accidents and injuries and maintaining safe conditions for patients, staff and visitors. * Develop and implement policies and procedures that guide and support the provision of the services. * Develop disease specific protocols for nursing to assist in supporting Best Practices for chronic care management. * Continuously assess and improve the department's performance by designing operational processes, monitoring performance through data, analyzing the data, implementing sustainable performance improvement, and participating in multi-disciplinary inter-departmental performance improvement activities. * Manage information by providing information systems and data management for department operations and performance improvement. * Oversee internal peer audits, processes to support organizational compliance. * Perform chart audits for all disciplines of the IDT in the organization, and adherence to care plan * Assumes organizational responsibility in the absence of the President/CEO; takes administrative on-call; maintains 24 hour accountability for departments within scope of responsibility * Is a member of the l ...
