Concordia Care, is a Program of All-inclusive Care for the Elderly (PACE site), whose mission is to provide an alternative to nursing home placement by enabling seniors to remain in the community.RECREATION THERAPISTWe are seeking a qualified candidate who will be responsible for ensuring effective and efficient implementation of participant therapeutic recreational activities for the organization with a focus on maintenance and restoration through the use of drama, nature study, social activities and arts/crafts developed to effect improvement in the well being of prorgram participants.  Incumbent will serve as an active member of the Interdisciplinary team and maintain an active caseload.Job Summary:   1. Develops and implements a coordinated, multi-faceted recreational therapy program for frail older adults including those with dementia, vision/hearing and/or physical impairments, or mental illness. 2. Ensures the implementation of participant activities by: a) supervising an ongoing program of individual and group participant activities which are appropriate to the needs of each participant b) supervising the development of an individual activities plan for each participant to include reviewing the plan and goal and recording progress notes as needed c) assessing individual participants for plan of care goals and objectives and formal reassessment and evaluation of the plan on a semi-annual and annual basis, or more often as necessary. 3.  Performs evaluation of participants by obtaining all pertinent information including psychosocial and medical history from chart, appropriate staff, and participant/family interview.4.  Matches participant's leisure and/or social recreational interests with appropriate group and/or individual therapy.5.  Attends and participates in meetings as an active interdisciplinary member of the team.6.  Develops and incorporates participant and family education into therapy programs.7. Actively participates in appropriate discipline-specific or interdisciplinary performance improvement activities through data collection, data analysis, and program improvement, among other functions.8.  Ensures compliance with individual, team, and/or discipline-specific financial and productivity target measures.9. Contributes to participant safety by providing a safe environment for participants, reporting safety concerns and participating in quality improvement initiatives10. Ensures compliance with the PACE regulations, industry standards and Concordia Care policies.11.  Supervises assigned personnel by a) providing guidance, assistance and training as necessary, b) overseeing scheduling and coverage of staffing and c) making recommendations regarding employment, performance appraisal, salary changes, promotions, transfers and terminations.  Interviews, hires, trains and ...
