Job Description:
Immediate Opening for an LPC, LMFT, FNP, Physician, Physician Assistant, Psychologist, or Psychiatrist. The Team Leader is responsible for coordinating the initial assessment and developing the Person Centered Plan and for providing or coordinating treatment (with another licensed professional) for the youth or other family members Services are structured and delivered face-to-face. Services will provide support and guidance in all functional domains. Intensive In-Home Services is a time-limited intensive family preservation intervention intended to stabilize the living arrangement, promote reunification or prevent the utilization of out-of-home therapeutic resources for the identified youth through age 20.
