We are seeking a Case Manager to join our team!
Provides services to consumers and their treatment teams to support achievement of psychiatric rehabilitation/recovery goals and objectives. Responds to crisis calls following all agency and contract procedures. Provides crisis stabilization and support with the individual, families and significant others as appropriate. Ability to develop and maintain therapeutic relationships with consumers and significant support systems. Demonstrates the ability to work with frequent interruptions, urgency of issues and interactions with a wide variety of professionals and personalities. Works as a member of a clinical team to assess consumers needs, strengths and goals in accordance with established policies and procedures and case management protocols. Engages in Family Centered, strengths-based service planning and coordination of services for consumers with crisis and treatment teams. Provides information, education and guidance to consumers and support system members concerning issues of psychiatric rehabilitation, treatment planning, advocacy, community services, and coordination of care.
Requires Bachelor’s degree in a social science or equivalent combination of education, training and/or experience preferred.
Knowledge of the principles of psychiatric problem solving and intervention techniques. Knowledge of DSM IV-TR diagnostic criteria, monitors mental status of consumers and response to treatment plans. Knowledge of the principles and strategies of psychiatric rehabilitation, readiness assessment/development and the implementation of psychiatric recovery plans.
