VISITING NURSE ASSOCIATION Visiting Nurse Association HouseCalls program delivers primary medical care and consultation to persons who have difficulty getting to the doctor's office due to a chronic, progressive disease, or from sudden, escalating symptoms of an illness. The Advanced Practice Nurse provides direct care and services to patients, maintains an active collaborative relationship with HouseCalls physicians, and is responsible for the medical management of patients. NURSE PRACTITIONER/CERTIFIED NURSE SPECIALIST IMMEDIATE OPPORTUNITY - PART TIME & PRN Master's of Science in nursing and board certification in Adult or Family Advance Practice Nursing. Current Certificate of Authority; prescriptive authority preferred. Minimum of five years clinical experience in nursing or as an APN. Experience in hospice or home care field preferred. Current CPR certification. Current driver's license and auto insurance. Knowledge and expertise in the management of elderly patients with chronic progressive illnesses with an emphasis on pain and symptom control, psycho-social management, and disease management.
