Description:
The Care Coordinator supports and coordinates mental health care to patients in the primary care clinics. You will have the opportunity to screen and assess patients for mental and substance abuse disorders, and monitor their on-going status. You will be a core member of a collaborative care team that involves the patient’s primary care provider, a consulting psychiatrist and other mental health providers. You will also be responsible for coordinating referrals to clinically indicated services outside the primary care clinic.
The GA-U Mental Health Integration Program is being implemented statewide to expand mental health benefits to a population in need of better coordination of care and more access to services that will help with recovery. Patients will benefit from a medical home, integrated services and faster progress toward either gainful employment or a determination of a more on-going need for support.
Requirements
Master’s degree in social work or counseling field required, and prior experience in a primary care setting or community mental health center preferred.
